Personal Emergency Evacuation Consent Form

Why We're Asking

This form is how we let the London Fire Brigade know who may need help to leave the building in a fire. The information you give us is held securely and, with your consent, shared with the Fire Brigade so that if they ever attend your development, they already know where you are and what support you might need - before they arrive.

Please complete and return the form as soon as possible, keep a copy for your records, and resubmit if anything changes. The survey is reissued annually so the information stays up to date.

Do you need a Personal Emergency Evacuation Plan (PEEP)?
A PEEP may be relevant to you if you have a physical disability, a visual or hearing impairment, a cognitive condition such as dementia, a temporary condition such as a recent injury or pregnancy, or any other circumstance that might affect your ability to leave the building without assistance.
Do you give consent to share your PEEP information with the Fire Brigade
